Difference between Cushing Disease and Cushing Syndrome

While they sound nearly identical and share the same symptoms, the difference lies entirely in the source of the problem. Think of "Syndrome" as the broad umbrella and "Disease" as one specific cause under that umbrella.

The Core Difference

  • Cushing Syndrome: This is the general term for the condition caused by having too much cortisol in the body for a long time, regardless of the cause. It is the "what."

  • Cushing Disease: This is a specific type of Cushing Syndrome caused by a pituitary tumor (usually benign) that produces too much ACTH (adrenocorticotropic hormone), which then tells the adrenal glands to pump out excess cortisol. It is a specific "why."

Comparison at a Glance

FeatureCushing SyndromeCushing Disease
DefinitionAny state of chronic cortisol excess.Cortisol excess caused specifically by a pituitary tumor.
CausesMedication (prednisone), adrenal tumors, or "ectopic" tumors (e.g., in the lungs).A benign tumor (adenoma) on the pituitary gland.
PrevalenceMore common (especially the medication-induced version).Represents about 70% of spontaneous (internal) cases.
Hormone levelsACTH levels may be low if the cause is adrenal or medication.ACTH levels are high, originating from the pituitary.

Why does the distinction matter?

The distinction is critical because the treatment is completely different:

  • For Syndrome: If it’s caused by medication, the doctor slowly tapers the dose. If it’s an adrenal tumor, they surgery focuses on the adrenal glands (located above the kidneys).

  • For Disease: Treatment almost always involves neurosurgery to remove the tiny tumor from the pituitary gland at the base of the brain.

Key Takeaway: All people with Cushing Disease have Cushing Syndrome, but not everyone with Cushing Syndrome has the Disease.
